Use modifier 77 when a repeat procedure is performed by a different provider than the one who originally performed it, on the same date of service. This may arise in group practice or collaborative care settings where a second clinician provides an additional therapy session. Both providers' information must appear on the claim and clinical documentation must justify the repeat visit.
