Place of Service Code 12: Home
What is Place of Service Code 12?
POS 12 identifies services rendered in the patient's own residence when the provider physically travels to the patient's home to deliver care in person. This distinguishes mobile or home-visit therapy from telehealth (POS 10) and standard office visits. It is used for community-based and in-home therapy programs.
When to Use POS 12
Use POS 12 when you physically travel to the patient's home to provide an in-person therapy session. This is distinct from POS 10, which is reserved for telehealth sessions where the patient happens to be at home.
