Out-of-network therapy can be expensive — but many insurance plans cover 50-80% of the cost if you submit a superbill. Here is how to do it.
Step 1: Check Your Out-of-Network Benefits
Call the member services number on your insurance card and ask: "What are my out-of-network mental health benefits?" Key questions: deductible, coinsurance percentage, and whether you need pre-authorization. For a full breakdown of what these terms mean, see deductibles vs copays explained.
Step 2: Request a Superbill From Your Therapist
Ask your therapist to generate a superbill after each session. It should include their NPI, your diagnosis code, the CPT code, and the fee paid. For the full list of required fields, see required fields on a superbill.
Step 3: Submit the Claim
Log in to your insurer's member portal, find the "Submit a Claim" section, and upload the superbill PDF. Some insurers accept faxed claims as well. For a detailed walkthrough of the process, see the step-by-step submission guide.
