Provider Fields
- Full legal name or practice name — must match your NPI registration
- NPI number — 10-digit National Provider Identifier, required for all claims
- Tax ID / EIN — for payment routing
- License number and state — e.g., LCSW-12345, CA
- Taxonomy code — 10-character code identifying your provider specialty
- Practice address and phone number
- Provider signature
Patient Fields
- Patient full name
- Date of birth
- Insurance member ID
Service Fields
- Date of service — must be exact, not a range
- CPT code — procedure code (e.g., 90837 for 60-min psychotherapy)
