Provider Fields
- Full legal name or practice name — must match your NPI registration
- NPI number — 10-digit National Provider Identifier, required for all claims
- Tax ID / EIN — for payment routing

Patient Fields
- Patient full name
- Date of birth
- Insurance member ID
Service Fields
- Date of service — must be exact, not a range

- ICD-10 code — diagnosis code (e.g., F41.1 for generalized anxiety)
- Fee charged — your full rate, not the discounted amount
- Amount paid — what the patient actually paid
- Place of service code — 11 (office) or 02 (telehealth)
