Why Each Field Matters

The NPI Number

The NPI is a unique 10-digit identifier assigned by the federal government. Insurance companies use it to look up the provider in their databases. If the NPI on the superbill does not match what is registered with NPPES, the claim will be rejected immediately — even if every other field is correct.

The Taxonomy Code

The taxonomy code identifies the provider's specialty. For a Licensed Professional Counselor it is typically 101YM0800X. For an LCSW it is 1041C0700X. Some payers use this code to determine whether your license type is a covered provider for the plan.

The CPT Code

CPT 90837 tells the insurer that a 60-minute individual psychotherapy session was delivered. Using the wrong code — for example, billing 90837 for a 45-minute session — is a billing error that can trigger audits. Always match the code to the actual time face-to-face with the client.

The ICD-10 Code

The ICD-10 code is the formal diagnosis. Without it, the insurer has no covered reason to pay the claim. The diagnosis must match your clinical records — it cannot be invented for the purpose of getting reimbursement.

Place of Service

POS 11 means the session took place at the provider's office. If this had been a telehealth session with the client at home, the code would be POS 10. A mismatch between the POS code and the session type documented in your notes is a compliance problem.

Fees and Payment

The insurer uses the "fee charged" line to compare against their Usual, Customary, and Reasonable (UCR) rate for the CPT code in your area. They will reimburse the lower of the two figures, minus the client's deductible and coinsurance responsibility. If the client paid less than your full fee (e.g., on a sliding scale), document both figures accurately.

What Happens After the Client Submits

Once your client submits this superbill to their insurer — typically through the member portal or by mail — the insurer will:

  1. Verify the provider's NPI and taxonomy code
  2. Check that the diagnosis code is covered under the plan
  3. Apply the client's out-of-network deductible and coinsurance
  4. Issue an Explanation of Benefits (EOB) showing what will be reimbursed
  5. Mail or direct-deposit the reimbursement to the policy holder
