A superbill is an itemized receipt that your healthcare provider gives you after a session. Unlike a standard invoice, it includes the specific medical codes insurers need to process a reimbursement claim.
Why Therapists Issue Superbills
When a therapist is out-of-network with your insurance, they cannot bill your insurer directly. Instead, you pay out-of-pocket and submit a superbill to your insurer yourself to get partially reimbursed — often 50-80% of the session cost, depending on your plan. What a Superbill Must Include
- Provider name, NPI number, and EIN/Tax ID
- License number and state
- Practice address and phone number
- Patient name, date of birth, and insurance ID

- CPT code — the procedure code (e.g., 90837 for 60-minute psychotherapy)
- ICD-10 diagnosis code — e.g., F41.1 for generalized anxiety disorder
- Fee charged and amount paid
- Place of service code — 11 (office) or 02 (telehealth)
- Provider signature
