What Is a Superbill? A Complete Guide for Therapy Clients

Learn what a superbill is, what information it must contain, and how to use it to get reimbursed by your insurance for out-of-network therapy.

A superbill is an itemized receipt that your healthcare provider gives you after a session. Unlike a standard invoice, it includes the specific medical codes insurers need to process a reimbursement claim.

Why Therapists Issue Superbills

When a therapist is out-of-network with your insurance, they cannot bill your insurer directly. Instead, you pay out-of-pocket and submit a superbill to your insurer yourself to get partially reimbursed — often 50-80% of the session cost, depending on your plan. To understand the difference between OON and in-network coverage, see in-network vs out-of-network therapy.

What a Superbill Must Include

  • Provider name, NPI number, and EIN/Tax ID
  • License number and state
  • Practice address and phone number
  • Patient name, date of birth, and insurance ID
  • Date of service
  • CPT code — the procedure code (e.g., 90837 for 60-minute psychotherapy)
  • ICD-10 diagnosis code — e.g., F41.1 for generalized anxiety disorder
  • Fee charged and amount paid
  • Place of service code — 11 (office) or 02 (telehealth)
  • Provider signature

How to Get Reimbursed

  1. Request a superbill from your therapist after each session (or monthly)
  2. Log in to your insurance member portal and find the claim submission form
  3. Upload the superbill and submit
  4. Wait 2-6 weeks for an Explanation of Benefits (EOB)
  5. Receive a check or direct deposit for the reimbursable portion

How Superbilled Helps

Superbilled generates insurance-compliant superbill PDFs in under 60 seconds. Therapists enter the session details once, and their clients receive a professional PDF they can submit directly to insurance.
