How to Submit a Superbill to UMR (Step-by-Step)

Check the Plan Has Out-of-Network Benefits First
UMR does not sell one standardized health plan. It administers claims on behalf of the employer that actually funds the benefits, and that employer decides what the plan covers โ including whether out-of-network mental health care is reimbursed at all, and at what percentage. Two clients who both carry a UMR ID card can have very different OON benefits depending on which company they work for.
Before you or your client assumes a superbill will lead to reimbursement, confirm the plan actually has an out-of-network benefit. What the Superbill Must Include
UMR reviews claims the same way any other administrator does: a superbill missing a required field is the single most common reason a claim gets suspended for more information rather than paid outright. Make sure your superbill has every item on our required superbill fields checklist, including:
- Provider name, credentials, and NPI
- Provider Tax ID (EIN or SSN)
- Practice address and phone number
- Client name and UMR member ID
- Date(s) of service, with each session itemized separately
- CPT code (e.g., 90837 โ Psychotherapy, 60 minutes)
- ICD-10 diagnosis code (e.g., F41.1)
- Place of service code (11 for in-office, 10 or 02 for telehealth)
- Amount charged and amount already paid by the client
Submitting Through umr.com
UMR's fastest option is its member portal. After signing in at umr.com with the information on the ID card, the portal generally routes members through a Claims menu to an online claim submission tool โ the exact labels and number of clicks can vary by plan and by whether the member is on the desktop site or the UMR mobile app, so treat this as a general path rather than a fixed script:
- Sign in at umr.com (or the UMR app). Members register a member account using the information on their ID card the first time they use the portal.
- Find the online claim submission option under Claims. Most plans surface this as some variation of "submit a claim" or "submit an online claim" โ if it isn't obvious, the portal's help section or member services can point to the right screen.
- Enter the service details and attach the superbill. You will be asked for the provider's information, dates of service, and diagnosis and procedure codes โ all of which should already be on the superbill you provide. Upload the superbill as the supporting documentation.
Because an out-of-network therapist typically cannot submit the claim on the client's behalf, this step falls to the member (your client). Once you hand over a complete superbill, the rest of the process is theirs to run.
Submitting by Mail
If your client prefers a paper claim, UMR's Member Medical Claim Submission Form is available from the "Form Center" on umr.com. The member fills in their name, member ID, and a brief description of the service, then attaches the superbill (an itemized statement of the charges) to the back of the form.
Mailing addresses and fax numbers for UMR claims vary by employer plan, so the safest source is always the claims address printed directly on the back of the member's ID card โ do not assume every UMR plan routes mail to the same P.O. box. If the card does not list an address, the member should call the customer service number on the card and ask where to send a paper claim for their specific plan.
Timeline and What Reimbursement Looks Like
Because UMR administers plans for many different employers, there is no single company-wide timely filing deadline or turnaround time โ the employer's plan document sets both. In practice, most UMR plans process a clean OON claim within a few weeks, but deadlines for submitting the claim in the first place can range from as short as 90 days to as long as a year from the date of service, depending on the plan. Confirm the specific deadline with member services rather than assuming it matches a plan your client had previously.
After the claim is processed, the member will receive an Explanation of Benefits (EOB) showing the allowed amount, how much of the OON deductible was applied, the coinsurance percentage, and the amount UMR is reimbursing directly to the member.
If the Claim Is Denied
A denial from UMR is not necessarily the end of the road. Common, fixable reasons include a missing NPI or Tax ID, a mismatched member ID, or a plan that genuinely excludes OON behavioral health.
