Health Net, a subsidiary of Centene Corporation, serves members primarily in California and select other states. OON mental health benefits vary significantly by plan type. PPO members can submit superbills via the Health Net member portal at healthnet.com or by mailing a claim form with the superbill to the OON claims department. HMO members generally have no OON coverage outside of emergencies.

• ✗Member has an HMO plan with no OON behavioral health benefits
• ✗Provider is not licensed in California or the relevant service state
• ✗Claim submitted without the required prior authorization for outpatient mental health services
• ✗Superbill does not include the member's full Health Net ID number
