New Jersey is one of the highest-rate OON therapy markets in the country, largely because of its proximity to New York City. Bergen, Essex, and Morris county therapists regularly command session fees of $175–$225, and Horizon Blue Cross Blue Shield of New Jersey — the state's dominant insurer — processes more OON mental health claims than any other carrier in the state.
New Jersey License Types and Billing
The New Jersey State Board of Marriage and Family Therapy Examiners, the State Board of Social Work Examiners, and the State Board of Psychological Examiners oversee the following credential types that can independently bill insurance:
- LPC— Licensed Professional Counselor (NJ's fully independent counselor credential)
- LCSW — Licensed Clinical Social Worker
- LMFT — Licensed Marriage and Family Therapist
- Licensed Psychologist — Doctoral level
New Jersey uses "LPC" for its independent counselor credential. Licensed Associate Counselors (LAC) are supervised and cannot independently bill. Your NJ license number, NPI, and taxonomy code must appear on every superbill.
Out-of-Network Therapy in New Jersey: The Landscape
New Jersey's OON therapy market is heavily influenced by NYC proximity. Bergen County (directly across from Manhattan) and the NYC commuter corridor (Hudson, Essex, Morris, and Union counties) support the state's highest fees. Many NJ-licensed therapists see clients who commute into NYC but prefer to see therapists closer to home. The Princeton/Mercer County area has a strong academic and pharmaceutical industry demographic. South Jersey (Camden, Cherry Hill) is influenced by the Philadelphia market.

- Horizon Blue Cross Blue Shield of New Jersey — The dominant commercial insurer in NJ with approximately 3.8 million members. Horizon offers multiple PPO products (Horizon PPO, Horizon Direct Access PPO, Horizon Omnia) with varying OON benefit structures. Horizon PPO plans include OON mental health benefits; Horizon Omnia is a tiered network plan where OON benefits are significantly reduced. Clients submit via the Horizon member portal at horizonblue.com. Processing typically 3–5 weeks.
- Aetna — Significant employer-group presence across NJ. Standard OON PPO benefits; clients submit via the Aetna member portal.
- UnitedHealthcare — Large employer-group presence, especially in corporate NJ (pharmaceutical and financial sector employers). OON claims through myuhc.com.
- Cigna / Evernorth — Common in NJ pharma and tech employer plans. PPO plans include OON mental health benefits.
- Oxford Health Plans (UHC) — Oxford is a UHC-owned plan that is common in the NY/NJ metro area, particularly for small-business employer groups. Oxford PPO plans include OON benefits; submit via myuhc.com.
- New Jersey FamilyCare (Medicaid) — No OON psychotherapy benefit. Medicaid managed care organizations (Horizon NJ Health, Aetna Better Health of NJ) contract with in-network providers only.

State-Specific Billing Regulations
- Mental health parity. New Jersey has one of the strongest state mental health parity laws in the country (NJ Mental Health Parity Act), which predates federal MHPAEA and in many respects exceeds it. NJ requires parity for a broad list of mental health conditions and has strong enforcement via the NJ Department of Banking and Insurance (DOBI). Clients can file complaints at nj.gov/dobi.
- Telehealth parity. NJ enacted telehealth parity legislation requiring commercial insurers to cover telehealth behavioral health services at parity with in-person services. Use Place of Service 10 for patient-home sessions. NJ also requires audio-only telehealth coverage for behavioral health.
- Horizon Omnia tiered plans.Horizon's Omnia plans significantly reduce OON benefits relative to traditional PPO plans. Clients with Omnia plans may face much higher out-of-pocket costs for OON therapy. Always ask clients to verify whether they have a traditional PPO or an Omnia tiered plan.

Tips for New Jersey Therapists
- Bergen County supports NYC-level fees. Bergen County therapists (especially in Ridgewood, Westfield, Montclair, and Hoboken) regularly set fees at $185–$230 per session. These markets are not price-sensitive — they are quality-sensitive. Invest in your credentials, niches, and online presence.
- Distinguish Horizon PPO from Horizon Omnia.This is the single most common billing confusion in NJ. Omnia plans reduce OON benefits to near zero in some cases. If a client has Horizon, always ask them to read their Summary of Benefits for "out-of-network" mental health coverage or call the member services number on their card.
- Oxford Health in small-business plans.Oxford (UHC) is particularly common in small-business employer plans in NJ. It's a UHC subsidiary but processes claims slightly differently. Claims submit through myuhc.com the same as UHC directly.
- NJ parity law is a strong client tool.New Jersey's parity law is significantly stronger than many clients realize. If a client is denied an OON claim or gets an unreasonably low reimbursement, a DOBI complaint is a legitimate and often effective step.
- South Jersey clients may have PA plans. Many South Jersey residents work in Philadelphia and carry Pennsylvania-domiciled employer plans through Independence Blue Cross or Highmark. These plans are governed by Pennsylvania insurance law, not NJ law.
