North Carolina License Types and Billing

The North Carolina Licensing Board for Licensed Clinical Mental Health Counselors, the NC Social Work Certification and Licensure Board, and the NC Marriage and Family Therapy Licensure Board oversee the following credential types that can independently bill insurance:

  • LCMHC — Licensed Clinical Mental Health Counselor
  • LCSW — Licensed Clinical Social Worker
  • LMFT — Licensed Marriage and Family Therapist
  • Licensed Psychologist — Doctoral level, NC Psychology Board

Note that NC uses "LCMHC" rather than LPC or LPCC for its independent mental health counselor credential. Your NC license number, NPI, and taxonomy code must appear on every superbill.

Out-of-Network Therapy in North Carolina: The Landscape

The Research Triangle (Raleigh, Durham, Chapel Hill) is North Carolina's fastest-growing OON therapy market, driven by a tech and biotech professional workforce with employer-sponsored PPO plans. Charlotte has a large financial services and corporate sector supporting premium OON practices. Greensboro, Winston-Salem, and Asheville are secondary markets with meaningful OON demand. Asheville in particular supports premium rates given its wellness culture and affluent demographic.

  • Blue Cross and Blue Shield of North Carolina (Blue Cross NC) — The dominant commercial insurer in NC with approximately 3.8 million members. PPO plans (Blue Options PPO, Blue Value PPO) include OON mental health benefits. Clients submit superbills via the Blue Connect member portal or by mail. Processing time typically 3–5 weeks.
  • UnitedHealthcare — Large employer-group presence in the Triangle and Charlotte. OON claims through myuhc.com; standard PPO OON mental health benefits.
  • Aetna — Common in corporate Charlotte and Research Triangle employer plans. OON PPO benefits; clients submit via the Aetna member portal.
  • Cigna / Evernorth — Present in Triangle and Charlotte corporate plans. PPO plans include OON mental health benefits.
  • WellCare (NC Medicaid Managed Care) — WellCare is one of the NC Medicaid managed care organizations. Medicaid plans do not offer OON psychotherapy benefits.
  • NC Medicaid / Tailored Plans — No OON benefit for psychotherapy. Tailored Plans (for individuals with behavioral health needs) are in-network only.

State-Specific Billing Regulations

  • Mental health parity. North Carolina enforces the federal MHPAEA. The NC Department of Insurance handles parity complaints; clients can file at ncdoi.gov.
  • Telehealth parity. NC enacted telehealth parity legislation requiring commercial insurers to cover telehealth behavioral health services. Use Place of Service 10 on superbills for sessions where the client is at home. North Carolina also requires audio-only telehealth coverage for behavioral health when video is inaccessible.
  • LCMHC independent practice. The LCMHCC (Licensed Clinical Mental Health Counselor Associate) is a supervised credential and cannot independently bill commercial insurance. Only the LCMHC designation permits independent billing.

  • Triangle OON market is underserved.Despite the Research Triangle's rapid growth, the number of OON-only therapists has not kept pace with demand. Therapists in Raleigh, Durham, and Chapel Hill who establish OON practices can typically fill panels quickly.
  • Blue Cross NC dominates — know their plans.Blue Options PPO offers meaningful OON benefits; Blue Value and ACA marketplace Blue plans vary widely. Always ask clients to confirm "PPO" plan type and whether mental health OON coverage is included before counting on reimbursement.
  • Asheville wellness market.Asheville's wellness and integrative health culture supports premium OON fees. LMFTs and somatic therapists in Asheville can charge at the high end of NC ranges ($160–$200+).
  • Charlotte financial sector plans. Many Charlotte clients work in finance and carry employer-sponsored plans from national carriers (Aetna, UHC, Cigna) with strong OON benefits. These clients are particularly receptive to OON practice.
  • Use Place of Service 10 for telehealth. NC Blue Cross NC and other carriers in the state are aligned on POS 10 for patient-home telehealth. Using the outdated POS 02 may cause claim delays.
