Cigna — now branded as The Cigna Group, with behavioral health services operating under the Evernorth name — makes it relatively straightforward to submit out-of-network therapy claims through the myCigna member portal. This guide walks you through the process from start to reimbursement.

Overview: Cigna, Evernorth, and OON Therapy

Cigna is a major national insurer. Its behavioral health arm, Evernorth (formerly Cigna Behavioral Health), administers mental health and substance use benefits for most Cigna commercial plans. When you submit a superbill for therapy, the claim is processed by Evernorth on Cigna's behalf — but you do everything through the same myCigna portal.

Most Cigna PPO plans cover out-of-network therapy after you meet your OON deductible, at which point the plan pays a percentage (typically 50–80%) of the allowed amount. HMO plans generally do not cover OON care except in emergencies.

Step-by-Step: Submitting Through myCigna

  1. Obtain your superbill. Ask your therapist for a detailed superbill covering the sessions you want to claim. Verify it includes all required fields before submitting.
  2. Log in to mycigna.com. If you have not registered, you will need your Cigna member ID. After logging in, navigate to Claims in the top menu.
  3. Select "Submit a Claim." Cigna's portal allows you to file a reimbursement claim for OON services. Select Medical/Behavioral Health when prompted for the claim type.
  4. Enter provider and service details. Fill in your therapist's name, NPI, dates of service, CPT code(s), and ICD-10 code(s). Upload the superbill PDF as supporting documentation.
  5. Confirm and record the claim number. After submitting, Cigna assigns a claim number. Keep this number — you will need it if you follow up on status.
  6. Alternative: submit by mail. Download Cigna's Member Reimbursement Request form from mycigna.com and mail it with your superbill to the address on your ID card. Allow extra time for mailed claims.

What to Include on Your Cigna Superbill

  • Provider full name, NPI (Type 1), and practice address
  • Provider Tax ID (EIN or SSN)
  • Your Cigna member ID and group number
  • Date of service for each session
  • CPT procedure code (e.g., 90837 for 60-minute therapy, 90834 for 45 minutes)
  • ICD-10 diagnosis code (e.g., F41.1, F33.0)
  • Place of service code (11 for in-office; 10 for telehealth)
  • Amount charged per session and total amount you paid

Cigna's Processing Timeline

Cigna typically processes behavioral health OON claims within 30 days. You will see the claim status update in myCigna before the EOB is generated. Review your EOB to verify the allowed amount and coinsurance applied correctly. Reimbursement checks are mailed to your address on file.

Cigna's timely filing deadline for most commercial plans is 180 days from the date of service. Some plans allow 365 days — confirm with member services.

Common Issues With Cigna Claims

  • Evernorth routing: If you are directed to an Evernorth phone line or portal for behavioral health, this is expected — Evernorth is Cigna's behavioral health subsidiary and handles the processing.
  • Prior authorization: Some Cigna plans require prior authorization for ongoing therapy (typically after session 8 or 10). Check your plan documents or call member services before your therapist's 10th session.
  • Low allowed amount: Cigna sets its own allowed amounts for OON services. If the allowed amount is significantly lower than the fee charged, request a fee schedule review.

Tips for a Smooth Cigna Claim

  • Call the Evernorth/Cigna behavioral health number on your ID card before your first session to confirm OON mental health benefits, your deductible status, and any prior auth requirements.
  • Submit through myCigna rather than by mail — online submission is faster and provides an immediate confirmation.
  • If your claim is denied, Cigna is required to explain the reason in the EOB denial code. Most clerical denials can be resolved through a first-level internal appeal.

Superbilled generates superbills that contain every field Cigna and Evernorth need for clean claim processing. Once your therapist issues the superbill, use the myCigna portal to submit and track your reimbursement.
