Florida Therapist Billing: OON Superbills and Insurance

Florida-specific guide for LCSW, LMFT, and LMHC therapists — Florida Blue, Aetna, telehealth parity rules, and OON reimbursement rates by metro.

Florida has one of the largest therapy markets in the southeastern United States, with a diverse population spread across major metros and rural counties alike. Whether you practice in Miami, Tampa, or Tallahassee, understanding Florida's licensing landscape and insurance environment is essential for running a sustainable out-of-network practice.

Overview

Out-of-network billing in Florida follows the same federal framework as every other state, but Florida-specific licensing boards, dominant regional payers, and evolving telehealth rules create a distinct environment. This guide walks through what Florida-licensed therapists need to know to generate valid superbills and help clients maximize their OON reimbursement.

Florida Licensing Boards

Florida regulates mental health professionals through the Department of Health (DOH) and several specialized boards:

  • Board of Clinical Social Work, Marriage and Family Therapy, and Mental Health Counseling — oversees LCSW, LMFT, and LMHC licensure
  • Board of Psychology — licenses psychologists at the doctoral level
  • Board of Medicine / Board of Osteopathic Medicine — covers psychiatrists who also provide psychotherapy

The most common license types for OON psychotherapy billing in Florida are:

  • LCSW — Licensed Clinical Social Worker
  • LMFT — Licensed Marriage and Family Therapist
  • LMHC — Licensed Mental Health Counselor
  • Psychologist — Licensed under Chapter 490, F.S.

All four license types can independently bill commercial insurance for psychotherapy services. Your Florida license number, your NPI number, and the appropriate taxonomy code must appear on every superbill you issue.

Insurance Landscape

Florida's insurance market is competitive, with several large payers controlling the majority of commercial coverage:

  • Florida Blue (Blue Cross Blue Shield of Florida) — the dominant payer in Florida, with strong penetration statewide. PPO plans typically offer OON benefits; clients submit superbills via the member portal at member.floridablue.com or by mail. Processing time averages 4–6 weeks.
  • Aetna — significant employer-sponsored presence throughout Florida. OON claims submitted via the Aetna member portal or by calling the member services number on the insurance card. Aetna's OON deductibles reset January 1 each year.
  • UnitedHealthcare — common among large employers and Marketplace plans. Clients submit OON claims through myuhc.com. Note that UHC's OON reimbursement for mental health can run 10–20% below Medicare rates in some Florida markets.
  • Cigna — present primarily in South Florida. PPO plans include OON mental health benefits; clients use myCigna.com or mail claims to the address on the EOB.
  • Humana — strong in South Florida, particularly among Medicare Advantage enrollees. Commercial PPO plans have OON benefits; Medicare Advantage generally does not.

Medicaid in Florida (Medicaid Managed Care) does not provide meaningful OON benefits for psychotherapy. If your clients are on Medicaid plans like Sunshine Health or Molina Florida, they generally cannot submit superbills for reimbursement.

Telehealth Rules

Florida enacted telehealth parity legislation (HB 23) requiring commercial insurers to cover telehealth services for mental health. Key billing rules for Florida telehealth:

  • Use Place of Service 10 (Telehealth Provided in Patient's Home) on superbills when the client is at home during the session
  • Use Place of Service 02 only if the client is at a medical facility or other eligible originating site
  • Florida requires that telehealth providers be licensed in Florida when the patient is physically located in Florida — this applies to both in-state and out-of-state therapists
  • Audio-only sessions may be covered by some payers but billing them correctly requires checking each plan's telehealth policy; see our telehealth billing guide for modifier details

Reimbursement Rates

OON reimbursement in Florida varies by metro area and payer. Typical private-pay fee ranges and estimated OON reimbursement for a 60-minute session (CPT 90837):

  • Miami / Fort Lauderdale: Private pay $180–$280; OON reimburse ~$80–$140
  • Tampa / St. Petersburg: Private pay $150–$240; OON reimburse ~$70–$120
  • Orlando: Private pay $140–$220; OON reimburse ~$65–$115
  • Jacksonville: Private pay $130–$200; OON reimburse ~$60–$110
  • Tallahassee / Gainesville: Private pay $120–$180; OON reimburse ~$55–$100

These figures represent the insurance portion after the OON deductible is met. Clients typically receive 50–80% of the plan's "allowed amount" for OON services once their deductible is satisfied. Learn more about how insurance companies determine reimbursement.

Practical Tips for Florida Therapists

  • Verify benefits before the first session. Call the member services number on your client's insurance card and ask specifically about OON mental health benefits, the OON deductible amount, and whether prior authorization is required.
  • Include your full license number. Florida insurance companies often require the therapist's state license number (not just the NPI) to process OON mental health claims.
  • Issue superbills promptly. Most Florida payers require OON claims to be submitted within 12–24 months of the date of service. Issuing a superbill at the end of each month keeps clients on track.
  • Know the parity rules. Florida follows federal MHPAEA parity requirements. If a client's OON mental health deductible is higher than their OON medical deductible, that may be a parity violation worth reporting to the Florida Office of Insurance Regulation.
