Understanding the state's licensing requirements and dominant payers, particularly Highmark and Independence Blue Cross, is essential for PA therapists operating OON.

Pennsylvania therapists can build thriving out-of-network practices by issuing proper superbills and guiding clients through the reimbursement process. PA has solid parity protections and a commercial insurance market dominated by regional Blue Cross Blue Shield affiliates.

Pennsylvania Licensing

The Pennsylvania State Board of Social Workers, Marriage and Family Therapists and Professional Counselors and the State Board of Psychology regulate mental health professionals in PA. Primary license types for OON billing:

  • LCSW — Licensed Clinical Social Worker
  • LPC — Licensed Professional Counselor
  • LMFT — Licensed Marriage and Family Therapist
  • Licensed Psychologist — Doctoral level

Pennsylvania does not have a separate "Licensed Clinical Counselor" designation — the LPC is the highest independent practice level for professional counselors. Your PA license number, NPI, and taxonomy code must appear on every superbill.

Pennsylvania Insurance Landscape

Pennsylvania has two distinct BCBS affiliates that together cover a large portion of the state's commercial insurance market:

  • Highmark Blue Cross Blue Shield — Dominant in Western PA (Pittsburgh, Harrisburg, Lehigh Valley). PPO plans include OON mental health benefits. OON claims submitted via highmark.com member portal or by mail to the claims address on the card. Processing typically takes 30–45 days.
  • Independence Blue Cross (IBX) — Dominant in the Philadelphia five-county area. Keystone HMO plans have no OON benefit; Personal Choice PPO and IBX Select PPO plans do. Clients submit OON claims through ibx.com.
  • Aetna — Present in employer-sponsored plans statewide. Standard OON PPO benefits; clients submit via the Aetna member portal.
  • UnitedHealthcare — Common among large employers in Philadelphia and Pittsburgh. OON claims through myuhc.com.
  • Geisinger Health Plan — Regional HMO centered in central PA. Limited OON benefits; primarily relevant for Geisinger Gold Medicare Advantage plan members.

Philadelphia vs Pittsburgh Markets

Pennsylvania's two major markets have meaningfully different dynamics:

  • Philadelphia: Higher private-pay rates ($180–$280 per session); IBX PPO plans dominant; strong demand for trauma and anxiety treatment in Center City and Main Line suburbs
  • Pittsburgh: Somewhat lower private-pay rates ($150–$230 per session); Highmark dominant; strong academic medical community (Carnegie Mellon, Pitt) creates demand for specialized services
  • Central and Rural PA: Private pay $110–$160; OON deductibles can be high relative to income; telehealth is especially important in areas with therapist shortages

Telehealth Rules

Pennsylvania enacted telehealth parity requirements for commercial insurers covering mental health services. For OON superbill billing:

  • Use Place of Service 10 for client-in-home telehealth sessions
  • PA Medicaid has its own telehealth billing rules separate from commercial plans; OON superbills do not apply to Medicaid
  • Both synchronous video and audio-only telehealth may be covered depending on the plan; confirm with each payer

Tips for Pennsylvania Therapists

  • Distinguish IBX plan types. Independence Blue Cross's Keystone HMO plan has no OON benefit. Always ask Philadelphia-area clients whether they have a Keystone HMO or a Personal Choice PPO plan before assuming they can submit superbills.
  • Check Highmark's BlueCard program. Some Highmark members who work for national employers may have a BlueCard PPO plan rather than a local Highmark plan. BlueCard OON rules can differ from local Highmark plans.
  • Know PA's parity protections. Pennsylvania follows federal MHPAEA parity rules. Clients can file complaints with the Pennsylvania Insurance Department if they believe their plan is applying higher cost-sharing to mental health than to comparable medical services.
